This is almost always caused by using a USB-C to Lightning cable, or using a USB 3.0 (blue) port on your computer. Plug your USB-A cable into a standard USB 2.0 (black) port. If you are using an AMD-based computer, try using an Intel-based computer if available, as AMD USB controllers sometimes drop the connection during the exploit handoff.
